Senior Services

 

 

We provide custom-tailored services that enable older persons to live in their own homes with independence and dignity. Services are provided in the San Gabriel and Pomona Valleys. Urgent Case Management and Supportive Services

 

For many seniors, case management and supportive services can mean the difference between independent living or convalescent care. Urgent case management and supportive services can quickly stabilize living conditions and provide needed help so clients can live in their own homes with dignity and independence for as long as possible.

 

Case managers do this by conducting an in-home assessment, developing a care plan, then coordinating and monitoring services for the individual.

 

Case managers can arrange: In-home services like homemaking, housekeeping and caregiving; transportation; Medicare/medical services; bill payment; meals; vision and hearing services. Plus many more services!

 

 

Telephone Reassurance

 

Telephone reassurance is provided through phone calls to homebound older persons. The calls are a consistent point of contact, friendship, reassurance, and a source for information about programs and services.

 

 

Meals Delivered to the Home

 

The Home Delivered Meals Program provides healthy meals to homebound older persons. The meals can be hot lunches that are delivered each weekday, or can be frozen meals that are delivered in a package of seven once per week.

 

 

STRESS REDUCTION TOOLKIT FOR LATINO SENIORS

A "Stress Reduction Toolkit" in English and Spanish has been developed for Latino seniors by staff of the YWCA Intervale Senior Services Program.

The kit is designed to provide a structured framework for preventing and addressing mental health risk factors. It empowers elders with the information they need for protective factors such as building good relationships with peers and caring family members, increasing social supports, increasing life skills, developing coping skills, reducing immigrant-specific stress, and building problem solving skills.
