Housing for Homeless Families

 

Through First 5 LA funding we can provide assistance, information, and support to residents of the domestic violence shelter and other domestic violence survivors in securing safe, appropriate housing.  Working in conjunction with the case management and other housing staff, the YWCA assists survivors in developing the best possible housing plan and in carrying out that plan.  Our staff understands  housing subsidies and resources and are familiar with the Rapid Re-Housing model.

 

 

 

Frequent Users of Systems Engagement (FUSE) (10th Decile Project)

 

A partnership between the Pomona Valley Hospital Medical Center (PVHMC), Pomona Community Health Center (PCHC), the YWCA of San Gabriel Valley (YWCA-SGV), the National Health Foundation (NHF), and the Corporation for Supportive Housing (CSH) was formed to create the Medical Respite Care program.

Medical respite care is acute and post-acute medical care for homeless persons who are too ill or frail to recover from a physical illness or injury on the streets, but who are not ill enough to be hospitalized.  Without proper respite care for recovering homeless persons, there is a high incidence of extended hospital stays, unacceptable discharges of unhealthy, not well homeless to the streets, repeated hospital and emergency room visits, and poor health outcomes.   Although several cities in the United States have respite care as a part of their medical services system, health care and social service coalitions in Southern California have long identified respite care as a missing part of our health care continuum.  This project brings together several successful models to effectively assist and stabilize homeless patients, and at the same time reduce hospital and community costs associated with serving this population. The models to be utilized are: 1) the National Health Foundation Recuperative Care model; 2) the Tenth Decile Project, Frequent Users of Systems Engagement (FUSE) model; and 3) the Housing First, Supportive Housing model.

The YWCA participates in the FUSE component of the project.  The YWCA-SGV delivers comprehensive assessment and follow-up case management for high-need homeless.  The Navigator will focus on helping the client access permanent supportive housing, and will ensure that temporary housing is provided until placement is available.  The navigator will assess the client’s basic needs and available family/social support, then provide ongoing case management to help meet the client needs, including food, clothing, and other basic necessities as well as permanent supportive housing, medical care, and income stability.

The Navigator will assist clients with a permanent housing plan, in preparation for discharge.  Though there is a lack of long-term housing options for the homeless in the San Gabriel Valley, permanent housing with appropriate services will be the goal for each client.  The YWCA-SGV will create a network of housing resources and options for project participants in order to provide the best potential options, resources and opportunities, as well as work to develop additional temporary and permanent supportive housing in the project area.
